Checklists for Clients
Before you come in to recieve assistance from the Pantry/Gas Assistance program, or the AEP Neighbor to Neighbor program, be sure to check over the checklist. Links to printable copies are at the bottom of the page.
AEP Requirements Checklist: Neighbor to Neighbor

Bill has to have a current disconnect notice or electric shut-off

Bill has to be at least $100.00

Sincere Effort Payment - Client needs to have paid at least $150.00 towards their electric bill in the past 90 days; where they can provide a receipt, a confirmation number from AEP, or a payment history faxed to The Salvation Army. Client may not use agency to help with this $150.00 payment.

Client needs to contact Community Action (740.369.3184) to find out if they qualify for the following programs. (Make sure to tell Community Action you are applying for NEIGHBOR TO NEIGHBOR program.) If client is on, or has been on the following programs, then a start date must be given. If they are not eligible for the programs, then a denial date must be given. Once the information is given, Community Action will fax the information to The Salvation Army.

Social Security Card for every member of the household that is over 6 months old

Birth Certificates of Medical Cards for children.
